In this context risk is defined as uncertainty of outcome, whether positive opportunity or negative threat, of actions and events. (Cabinet Office 2002 page 7) Today it is recognized that public service organizations face many different types of risk across the whole spectrum of management activity including policies, programmes, operations, projects, finances, human resources, technology, health, safety, national and local politics, and organizational reputation.

Investing in ever-greater rail safety measures beyond what is reasonably practicable under the Health and Safety at Work Act, for example, would be likely to put up rail fares, and have implications for both access charges paid by train operators and Government subsidy. It could also lead to greater use of the roads. This would in turn lead to more transport-related deaths. (Better regulation task force 2003 page 20) However, the current approach to accountability used by government departments, combined with their inability to manage risk successfully, has been considered to be undermining innovation and change and, therefore, not able to deliver the expected outcomes required of modern public services.

This includes the creation of policies and procedures by the management of organizations, which may be used to ensure legislation is met or organizational policies are followed and is, therefore, a form of formal or objective control applied internally within organizations. Internal control in this form is the province of internal auditors who are employed purely to examine internal control processes. Managers and auditors alike are exhorted to identify adequate control, which in turn is defined in terms of the plans and organizational structures that provide reasonable assurance that organizational objectives will be achieved effectively and economically.
